Personal experience is that although agile and scrum are widely used, most organisations use these as frameworks to build a custom approach. Biggest problem I always find is lack of accounting and governance transformation to support agile, these two are always typically waterfall, so then you end up with agile technologists versus waterfall business = waterfall with sprints.
