When it comes to local searches, i recommend toggling the little switch on the left that limits searches to a specific country/language. I have found that this helps a lot.
The terms "gender" and "sex" are defined differently by different people, but in general the definition is something like this:
Sex is usually defined in terms of genitalia, genotype (chromosomes) or similar biological properties. However, this is still not a binary male/female property, see for example intersex people. Depending on your definition, undergoing surgery may change your sex, gender, or both.
Because the large majority of users have no idea that there are alternatives, or are scared to switch because they are so locked into the whole google ecosystem.

The site usually does not usually handle the serving of ads themselves. They often use a service like AdSense to outsource advertisements.
Just because a site has targeted ads doesnt automatically mean its bad.
