I looked at Runbox. Their offering does not satisfy my requirement "where price is not per account but per domain with unlimited or large number of accounts".
What they have is a main account with lots of aliases, but each separate user account, which they call "subaccount" is extra pay.
Would hosting your own be an option? You can get a VM in your cloud provider of choice for say 10 USD a month, then add a control panel such as hestia, ispconfig, mailcow (and many others), and then you have full control over your email. Add as many domains, email accounts as you like.
I should add that most of these options have installer scripts that take all the setup work out of the equation.
I am a software developer, so I guess I could have done it. That said, I'd rather not do it, spending my time on other things.

I would ideally have an email service separate from my domain web hosting. I would also need more than a couple of accounts some of the services provide (e.g. ZOHO)
